Operative
findings were consistent with the TEE: a

|
myxomatous
MV with bileaflet prolapse and a flail segment of
|
|
|
the
middle scallop of the posterior leaflet
|
|
|

The
anterior leaflet was resuspended with Gortex cords and
|
|
|
quadrangular
resection of the large middle scallop performed. A
|
|
36
mm annuloplasty ring was then sutured into place.
|
|
|
| • |
Cardiac
activity was restored and weaning from CPB started.
|
|
|
Attempts
to discontinue CPB resulted in severe systemic
|
|
|
hypotension
and distention of the heart.

examination of the mitral revealed the following.
